Doctor Relor,

You are informed of the imposition of restrictions to you movement by the Captain of the U.S.S. Independence. Until further amended, you are restricted to the ship, U.S.S. Independence, for a period of thirty days. During the period of the restriction your movements will be monitored by the ship's computer and you will be prevented from the following:

1. Accessing transporter rooms on U.S.S. Independence
2. Accessing airlocks on U.S.S. Independence
3. Accessing shuttlebay on U.S.S. Independence
4. Accessing remote transporter functions

Appeals of this restriction can be directed to the Fleet Inspector's General Office.
